2017 PRINTABLE REGISTRATION FORM for Grades K-5 Player/Volunteers

2017 PRINTABLE REGISTRATION FORM for Grades 6-8 Player/Volunteers

The 2017 Jensen Center Youth Soccer program will begin on Thursday April 20 and run through Saturday, May 20, 2017. Just as in past years, games will be played on Thursdays and Saturdays each week at the soccer fields behind the TR Schools and the Lettie Jensen Community Center. We are happy to welcome Amber Winkleman as this year’s Soccer Program Coordinator.

Teams will consist of divisions according to age. The divisions are: kindergarten through first grade; second and third grade; fourth and fifth grade, and sixth through eighth grade.

Close to 300 youth signed up for the soccer program last year. Early registration is highly encouraged to achieve a similar turnout for team selection and t-shirts for players. Online Registration begins on January 4, 2017. You may click on the above links for a registration form, print it out, and mail or return it in person to:
Jensen Community Center
487 North Main Street
Amherst WI 54406

Hard copies will be available if needed at the Jensen Center. The registration fee is $35 if paid by March 15, 2017. After March 15, the fee is $45 to help offset the extra costs of additional t-shirt orders, and time to re-group teams with late registrants. No child will be turned away because of financial difficulties. Please contact the Jensen Center for information about our confidential assistance program.

Game Schedule:

Grade K -1 Division - Thursdays at 5:45 p.m. and Saturdays at 10 a.m.

Grade 2 - 3 Division - Thursdays at 6:45 p.m. and Saturdays at 9 a.m.

Grade 4 - 5 Division - Thursdays at 6:45 p.m. and Saturdays at 9 a.m.

Grade 6 - 8 Division - Thursdays at 5:45 p.m. and 6:45 p.m. and Saturdays at 10 a.m.

Pre-K will also practice on Saturdays at 9 a.m.

We are currently seeking referees to hire. Each referee will be assigned to a specific field for the duration of the season, and will be paid in one payment at the end of the season according to the number of games refereed. Contact the Jensen Center for more information and an application.

The soccer program would not be possible without the help of volunteers. We are always looking for coaches, co-coaches, (paid) referees, field maintenance help, and ice cream social volunteers. Anyone interested in volunteering, becoming a referee, or having any other questions about the program should contact the Jensen Community Center 715-824-5202 during office hours (Monday-Thursday 9-4:30 and Fri 9-noon).
